Why self-management

Looking after yourself is one of the most important things you can do in your lifetime.

Your whanau/family, friends and future depend on it.

Self-management sessions mean you can take control and do something to reduce your risk of having a heart attack or stroke.

By making some lifestyle changes you can improve your heart health, and your health and wellbeing in general.

You don’t have to make big changes, small ones are a great start.

This programme can help you achieve the changes that are important to you, to help your heart now and in the future. Involve your partner, family/whanau in this programme too, their support can really help. Just two hours a week for four weeks can help you take back control of your own heart health.

What to expect

The Heart Health Self-Management sessions run for two hours per week, over a four week period.

They are FREE for both you and a support person to attend.

They are designed to provide information and education to improve your heart health and reduce your risk factors.
