At EBPHA, we provide and support primary health care services in our rohe (district).

Our first level primary health care is provided by 9 General Practices with 27 General Practitioners and 33 Practice Nurses, who serve 29,000 enrolled patients throughout the Eastern Bay of Plenty.

 It is our role to bring together GPs, nurses, and other health professionals, such as Māori health workers, health promotion workers, dietitians, counsellors, and psychologists, to serve the needs of our community.

While many of our services are directed towards individuals and illness, we are also focused on issues that impact on the wellness of specific groups within our communities, of whom 56% are Māori, 56% live in deprived circumstances, and 73% are “High Needs”.

 We are responsible for ensuring current challenges in community health care are being met. These challenges include equitable healthcare for Māori, child and adolescent health, immunisation, mental health, and long-term conditions such as diabetes and cardiovascular/heart disease.

Integrated Case Management

In consultation with you, your whānau, and your GP we provide Integrated Care Plans to help navigate the healthcare and social services system.

Childhood Immunisation Service

We support General Practices with their childhood immunisation programme, by providing an outreach service to our community.

Diabetes and Heart Health

We provide self-management sessions for you and your whānau, to help understand, manage, and improve your condition.

Youth Sexual Health

We support Eastern Bay of Plenty secondary schools with comprehensive youth sexual health education programmes.

Dietitian Services

We work with you and your whānau to  achieve and maintain a healthy lifestyle that’s right for you.

Hāpainga Stop Smoking Service

Quit smoking your way. We provide individual support or rōpu (group) sessions to help you quit.

Primary Health Counselling

We provide confidential, short term counselling support for people of all ages.

Healthcare in Schools

Our registered nurses provide school-based health services to Eastern Bay secondary schools.

Integrated Primary Mental Health & Addictions

We work to improve your overall health, supporting you with all aspects of your life.

Pasifika Case Management

We provide support for Pasifika fanau living in the Eastern Bay of Plenty.
