Primary Health Counselling / The counselling team provide short term support for those identified by the doctor with mild to moderate mental health concerns / conditions.
Hāpainga - Stop Smoking Service / Hāpainga is a FREE service to help people in the Bay of Plenty to quit smoking.
Outreach Immunisation Service / Outreach Immunisation Services (OIS) are for children and families/Whānau that are not accessing immunisation services provided by General Practice.
Heart Health Self Management / Self-management sessions mean you can take control and do something to reduce your risk of having a heart attack or stroke.
Diabetes Self Management / Self-management sessions mean you can take control and do something to ensure you remain well with your Diabetes.
Dietitian Services / The EBPHA Dietitian service provides one-on-one individual appointments and the dietary education component of the Diabetes and Cardiovascular Disease Self-Management Groups
Integrated Case Management Service / The ICM service supports General Practice in the care of patients with chronic care conditions and/or complex needs.
